By electronically signing this document, you are agreeing to the following regarding your child/student:

You, as a parent/guardian of your child, hereby authorize Science Kids to obtain medication or medical attention for your child in case of emergency if we are unable to reach you as a parent, guardian or physician stated above, and you release them from responsibility in connection with such medication or medical attention. During your child's attendance in the program, he/she shall be responsible for his/her own acts and save, indemnify and hold Science Kids harmless from death or injury to him/her or from damage to his/her property. Furthermore, by signing below, you are confirming that your child does not have a communicable disease, and that he/she is authorized to ride the Science Kids bus. During Science Kids activities, COVID-19 protocols will be followed by the participant and family members.

Science Kids personnel regularly administers sunscreen and botanically-based insect repellent to protect our students. If you do not want your student to have sunscreen or insect repellent applied by Science Kids personnel, please indicate these wishes in writing to the instructor or director prior to the start of the class. If you want your student to use DEET based repellent, you will need to provide it, and the student must apply it to himself or herself.

We regularly take photos of our students. Photos will only be used for Science Kids literature and promotion. If you do not want your student photographed please indicate your wishes in writing to the instructor or director prior to the start of class.
